2 sets of the following documents must be submitted:

Photographs of the inside and outside of the business premises (3 minimum), showing the complete front of the business premises with the Corporate Name visible, counter, office and merchandise in stock.

Copy of the Inscription to the Treasury Department (Inscription Application R1 and Complete Taxpayer’s Identification Number).

Copy of the Articles of Incorporation registered before the Public Registry of Property and Commerce.

Address evidence under the Name of the Business (Electricity, Water or Telephone Bill).

Two small (3 x 2.5 cm) photographs of each beneficiary.

Copy of 3 Invoices of purchases made in California with their corresponding Customs Declaration.

Copy of 3 Bills of sale issued by the Business.

Copy of its last tax payment before the Treasury Department.

Copy of the official identification of the applicant.

In case of applying for additional cards, attach Authorization Letter with the names of the authorized persons in headed paper of the company.
The State Board of Equalization retains the right to require additional documentation for authorization of the Card, as well as to give or reject said procedure.
